Willow Pediatric Wellness Online Appointment Booking - Terms of Service

Welcome to Willow Pediatric Wellness! These terms outline the policies regarding online appointment booking for non-members. By booking an appointment with us, you agree to the following terms:

1. Appointment Booking and Deposit

  • A $75 deposit is required at the time of booking to secure your appointment request.

  • The deposit will be applied toward your total service cost at the time of your visit.

  • Your appointment request is not guaranteed until you receive confirmation from our office of your appointment.

2. Service Fees

  • We do not accept insurance. All services are cash-based.

    • In-office urgent care visits: $135

    • In-home urgent care visits within our driving radius: $199

    • In-office sports/camp physical visits: $55 (group rates available for 4+)

    • In-home sports/camp physical visits: $115 (group rates available for 4+)

    • In-office medical grade ear piercing: $139 (group rates and home visits available for 4+)

    • Visits outside our driving radius are subject to travel fees, starting at $50, and will be evaluated on a case-by-case basis. You will be informed in advance if a travel fee applies and the amount of the fee. If you choose to cancel your appointment based on this information, your $75 deposit will be refunded.

  • Our urgent care services include:

    • Thorough evaluation, exam, assessment and treatment recommendations by our board certified pediatrician, Dr. Laura Mitchell.

    • Urine checks for urine infections and testing for strep, COVID, and flu.

    • Single-dose medications such as Zofran® (ondansetron), albuterol, and steroids.

    • If required, additional testing will be sent to our preferred partner lab, Core Diagnostics. Core Diagnostics will file your insurance for any services provided by their laboratory.

    • After your visit, a care summary will be sent to your child's primary care provider if you consent to sharing of your child's medical records.

  • Our Sports/Camp Physical services include:

    • Thorough evaluation, exam and assessment by our board certified pediatrician, Dr. Laura Mitchell.

    • Completion of any associated forms.

  • Our medical grade ear piercing services include:

    • Ear piercing in a safe environment by our board certified pediatrician, Dr. Laura Mitchell.

    • Piercing with medical-grade plastic earrings that are hypoallergenic, nickel-free.

  • Super bills are available upon request for urgent care visits and sports/camp physical, which you may submit to your insurance if you have out-of-network benefits. However, we do not guarantee or make any claims that you will receive any form of reimbursement for our services.

3. Pre-Assessment

  • Unsure if we can address your child’s concerns? You may text our office at 205-964-9575 prior to booking for a pre-assessment to determine whether your child’s complaint or condition is suitable for our care.

4. Payment Terms

  • Payment in full is required at the time of service.

5. Cancellation Policy

  • If you need to cancel or reschedule your appointment, we kindly request that you do so at least 24 hours in advance.

  • Cancellations made with less than 24 hours’ notice will result in the forfeiture of the $75 deposit, which will be non-refundable.

6. No-Show Policy

  • If you do not attend your scheduled appointment without prior notice, your $75 deposit will be forfeited as a non-refundable no-show fee.

7. Rescheduling

  • You may reschedule your appointment without penalty if done at least 24 hours in advance of your appointment time. Any requests to reschedule within 24 hours of your appointment will be treated as a cancellation and are subject to the cancellation policy outlined above.

8. Late Arrivals

  • We ask that you arrive on time for your appointment. If you are more than 15 minutes late, we may need to reschedule your visit, and the $75 deposit will be forfeited.

9. Emergent Services

  • Please note that Willow Pediatric Wellness does not provide emergent services. As a concierge-style practice, we are also not equipped the same as a traditional urgent care.

  • We do not provide IV fluids, blood work, x-rays, or imaging.

  • If your child requires emergency care, you are instructed to call 9-1-1 or visit the nearest emergency room.

10. Refunds

  • All services provided are non-refundable.

11. Changes to Terms

  • Willow Pediatric Wellness reserves the right to modify these Terms of Service at any time. Changes will be posted on our website, and continued use of our online booking system constitutes acceptance of the updated terms.
